- The consent form will be included in your child's file. It will be visible to the staff in the group and to you in the parent portal (Kindplanner).
- You are responsible for the accuracy of the information and permissions you provide in this form.
- The numbers in the form refer to the explanations below. Please read these explanations carefully before giving your permission or filling in any information.

Explanation per component
1. Photo and video recordings – public images
We sometimes take photos and videos of the group, on location, or during outings.
If you give your consent, we will use this visual material for our communication channels, such as door-to-door newspapers, websites, social media, posters, flyers, leaflets, and advertisements.
The material will be stored securely in a digital photo archive that only employees of the marketing department have access to.
The images will be deleted from other devices as soon as possible.
Would you like to give permission for us to use images of your child in this way? If so, please tick this box on the form.
The permission applies to the creation, storage, use, and deletion of images by Espira Kinderopvang.
During special activities, such as parties or outings, third parties (e.g., parents, grandparents, the press, or organizations hosting us) may also take photos or videos. The images they capture are not the responsibility of Espira Kinderopvang. By participating in these activities, you automatically agree to this.
2. Images for internal use
We sometimes take photos and videos in the group, on location, or during outings.
When time permits, staff members post this visual material in “Your Daycare” in the parent portal. This is a secure environment: only you and—if they appear in the same visual material—the parents of other children can view the images.
We remove the images from the devices used to take them as soon as possible. For this reason, you cannot request these images from us.

This consent applies to the creation, storage, sharing, and deletion of images by Espira Kinderopvang.
During special occasions, such as a summer party, outing, or swimming test, third parties (e.g., parents, grandparents, the press, or organizations where we are guests) may also take photos or videos. The images they capture are not the responsibility of Espira Childcare. By participating in these activities, you automatically agree to this.
3. Trips and transportation
Do you give your permission for transportation and trips, so that your child can join our trips and organized activities outside the childcare location? Also for taking your child to and from school to the childcare center?
If so, please check the options for this section on the form.
4. Going home from after-school care independently: independence contract
From the age of 9, your child may leave after-school care independently, provided you have given your consent via this consent form and the form has been fully processed and is visible to the group and to you in ‘My childcare’.
By giving this permission, we enter into a so-called independence contract with you.
This contract contains the following agreements:
As soon as your child is allowed to go home, you send a message to the group via ‘My childcare’.
A phone call is not sufficient for this.
As soon as we have received your message, we will prepare your child to leave the after-school care location.
When your child leaves the location, we will send you a confirmation via ‘My childcare’ that your child has left.
Our contractual responsibility ends the moment your child leaves the after-school care facility. From that moment on, you are responsible for your child, even if the care time specified in the contract has not yet ended.
The independence contract applies to contract days, exchange days, and extra days.

5. Playing outside independently (from age 7)
From age 7, your child may play outside independently at the after-school care location, without direct supervision by a pedagogical staff member.
We only do this if the pedagogical professionals deem it responsible, you have given your consent via this consent form, and the form has been fully processed and is visible to the group and to you in My Childcare.

6. Sleeping
Espira follows the national Safe Sleeping Protocol in childcare and GGD guidelines. This means that the following is not permitted during sleeping unless parents give their consent. By giving consent, the parent accepts full responsibility and liability for any incidents. Below is an explanation for each item.
A. With fixed jewelry
At Espira, children sleep without jewelry to prevent the risk of injury or suffocation. Small parts can come loose and pose a choking hazard. Neck jewelry can also be dangerous during sleep.
Do you still want your child to wear jewelry while sleeping? This is only possible if it cannot be removed without breaking and if you have given your express permission.
We always remove jewelry that can be removed without breaking.
B. Cuddly toy
Cuddly toys and loose objects are not allowed in principle. A small cuddly toy is allowed, for example, with a filled head and a layer of fabric.
C. Sleeping on the stomach (babies up to 1 year old)
If children under 1 year old sleep on their stomachs, there is an increased risk of cot death.
D. Swaddling
If parents give permission for their son or daughter to be swaddled.
E. Sleeping outside
In Scandinavian countries, it is common for children to sleep outside, and this seems to be becoming increasingly common in the Netherlands as well. Finnish research shows that sleeping outside has benefits for children, such as longer sleep duration and better sleep quality. Possible disadvantages are related to extreme outdoor temperatures and inappropriate clothing and bedding. The environmental factors in Finland differ from those in the Netherlands; in the Netherlands, we do not experience extreme outdoor temperatures and children can be dressed appropriately for Dutch temperatures.
Currently, more and more daycare centers are offering the option of letting children sleep outside. Espira Kinderopvang also offers this option. We have purchased special safe outdoor beds for this purpose, which comply with legal standards. Sleeping outside is only allowed in suitable weather conditions and under supervision.
You can give your consent by checking this box on the consent form. By checking this box, you declare that you understand the risks and will not hold Espira Kinderopvang liable for any consequences of sleeping with jewelry, a cuddly toy, sleeping on the stomach, or swaddling.

7. Nutrition
Soms mogen kinderen bepaalde voedingsproducten niet eten, bijvoorbeeld vanwege een allergie of geloofsovertuiging.
Is dat bij jouw kind het geval? Vul dit dan in op het formulier, zodat wij hier rekening mee kunnen houden.
8. Medication use and medical information
Does your child take medication or are there other medical issues we should be aware of? Our educational professionals ask parents to provide clear information about medication administration and responsibilities. Please consult with our daycare center if there are any changes in medication use.
Please briefly note the details in the field provided. We also ask you to record all agreements in writing so that we are fully informed and can take them into account. All information about the medication and its administration is recorded on a medication form.
We request that you complete and submit this medication form. This means that you actively sign in advance to give permission for the medication to be administered.
9. Information transfer to primary school
We do everything we can to make the transition to primary school as smooth as possible. We therefore ask you to complete and return this primary school transfer form. It helps the teacher to better understand the child and to tailor their teaching to his or her development. The form will be completed with care by your child's mentor. As a parent, you decide whether you want to hand this form over to your child's primary school.
10. Authority and pick-up by divorced parents
Als ouders gescheiden zijn, kan het voorkomen dat maar één van de ouders het kind mag ophalen. In dat geval kun je dit aangeven op het formulier. Wij handelen op basis van de informatie die jij aan ons doorgeeft.
Hebben we hierover geen informatie ontvangen? Dan mag je kind met beide ouders mee.
Als jij en je (ex-)partner hierover van mening verschillen, hebben we een gerechtelijke uitspraak of een deel van het ouderschapsplan nodig om de situatie goed vast te leggen en daarnaar te handelen.
Een uittreksel uit het gezagsregister is hiervoor niet altijd voldoende.
Ontvangen wij van jou een uittreksel uit het gezagsregister, dan handelen we daarnaar totdat je (ex-)partner aantoont dat hij of zij ook wettelijk gezag heeft.
Zodra dit is aangetoond, handelen we daarnaar en mag je kind ook met je (ex-) partner mee, ook als je op het toestemmingsformulier iets anders hebt ingevuld of een uittreksel uit het gezagsregister hebt ingeleverd.
11. Provision of information to divorced parents
If parents are divorced, we may only be allowed to provide information to one parent.
In that case, you can indicate this on the form. We will act on the basis of the information you provide us with.
If we have not received any information about this, we will provide information to both parents.
If you and your (ex-)partner disagree on this, we will need a court ruling or part of the parenting plan to record the situation and act accordingly.
An extract from the custody register is not always sufficient for this.
If we have an extract from the custody register and the other parent requests information about the child, we will ask this parent to prove their legal authority.
Once this has been proven, we will provide information to both parents, even if you have indicated otherwise on the consent form.
12. Pick-up by third parties
Only persons who have been notified in advance may pick up your child; identification is required.
